Over the past two weeks, we have explored various sections of our new website including our Sinus & Allergy page and our Snoring & Sleep Apnea page. This week, the next stop on our journey is our new Patient Information page. This page is specifically tailored for new patients as a valuable resource for your first appointment with the Pasha Snoring & Sinus Center. Like other pages on our website, the focus is YOU! We created this page for your convenience to save you time on the day of your appointment and so you know what to expect ahead of time. We also want to ensure that you have a seamless user-experience when you interact with us virtually which will mirror your in-office experience.

A link to the Patient Information page can be found at the very top of our homepage towards the right corner. Once you click the link, the Patient Information page is split up into four tabs: (1) Registration Process; (2) Patient Forms; (3) In Network Insurance Plans; and (4) Patient Downloads. We’ll look at each area, in turn, beginning with the Registration Process tab.

Registration Process

Once you have decided to visit the Pasha Snoring & Sinus Center for your health concerns, there are four main steps that you have to complete. These steps are listed in the Registration Process tab. The first step is scheduling an appointment. You have the option of doing so online or by calling 713-523-8800. As you scroll down the page, the second step reminds you to gather your insurance plan information (if you have insurance) and have it handy when setting up your appointment. The third step is filling out a few patient forms. We’ll touch on this in greater detail a little later. The fourth and final step is to try your best to obtain any prior studies and workup so you can bring this information to your appointment. Examples include sleep studies, imaging results, or any relevant prior health records. All of this information aids us greatly in establishing the right treatment plan for you.

We designed the Registration Process tab to serve as not only a portal but also as a resource for valuable health information. On the right side of the page, you’ll see some tips. These tips mention things you should do prior to your appointment as well as what you should expect and bring on the day of your actual appointment.

Patient Forms

To the right of the Registration Process tab is the Patient Forms tab. The five forms listed are: (1) Personal Health History Form; (2) Assignment of Benefits; (3) Financial Policy; (4) Notice of Privacy Policy Acknowledgment; and (5) Patient Consent for Disclosure. By including these forms online, you have the opportunity to get a jumpstart on paperwork prior to your appointment. This saves you the time of completing it while you are in the waiting room. For each form, there is an Online link and a PDF link. The Online link brings you to an online form on our website where you can type in the necessary information and submit it to us electronically. If you’d prefer to have a paper copy, the PDF option allows you to complete the same form by hand and then bring with you to your appointment.

In-Network Insurance Plans

To the right of the Patient Forms tab is the In Network Insurance Plans tab. There are 36 insurance plans that are listed including Aetna, Blue Cross Blue Shield, Humana, and more. We accept most private insurance plans, Tricare, and Medicare. We also accept some HMOs and IPAs, but you may need to obtain a referral from your primary care physician. If you do not have insurance, we may be able to provide assistance- please call us at 713-523-8800.

Patient Downloads

To the right of the In Network Insurance Plans tab is the final tab, Patient Downloads. Here you’ll find several documents that you may download as a PDF, including the following: (1) Dosing Schedule; (2) OR Consent Form; (3) PreOperative Guide; (4) Throat Surgery Post Op Instructions; (5) Nasal Surgery Post Op Instructions; (6) Home Immunotherapy; and (7) Nasal Irrigation Recipes. Note that some, if not all, of these documents, may not apply to your treatment. After your first appointment, we will let you know whether you should download any of the documents listed on this tab.
