1 PATIENT FORMS
Before your appointment, you’ll need to fill out a few forms acknowledging our privacy and financial policies and providing us with some of your information. You can save time by doing this now at the following link instead of in the waiting room.
2 ACCEPTED INSURANCE LIST
If you will be using your insurance, then gather your insurance plan information and card prior to setting up your appointment. We accept most private insurance plans, Tricare, and Medicare. We also accept some HMOs and IPAs, however, you may need to obtain a referral from your primary care physician. Please see link below for complete list of in-network insurance plans. to see our accepted insurance plans. If you do not have insurance, we may be able to provide assistance.
3 GATHER YOUR RESULTS
Please try to obtain any related prior studies and work-up including sleep studies, imaging results, or any relevant prior health records. It is best if you can bring the original films if you had an imaging done, especially CT scans.
- If you are taking an antihistamine such as Clartin™, Benadryl™, Xyzal ™, Zyretc™, Astelin™, Astepro™, Clarinex™, Allergra or over-the-counter cold remedies that contain an antihistamine; please stop the medications three days prior to your appointment if you anticipate allergy testing.
- On the day of your appointment be prepared to spend 1-2 hours for your appointment to allow for various testing and educational materials to be presented to you. Dr. Pasha does not like to have you wait, however, in the field of medicine it often is difficult to judge the timing of patient visits. We have reading material available but feel free to bring your I-pads and computers.
- Write down any specific questions or concerns you may have. Dr. Pasha loves prepared patients!
- Bring family members or friends. Our staff will spend time educating you on your condition and therapeutic options. It’s recommended to have someone else there to listen, discuss and help you make important health decisions.
